Schedule Performance Reviews for Individual Employees

I use Qualifications and set a review date within the qualification. Once reviewed I upload the documents outlining the review and outcomes etc. and this then sits on the employees profile as a record of the performance review having being conducted. The employee can access along with Managers. The only shortfall is you need to create many qualifications eg: Performance Review 1, 2, 3, 4 etc.. You can not add multiple documents to a qualification thereby requiring additional qualifications for each review period.
