Option to Have "Did Not Start" as a deactivation reason.

Could we keep the Reason box which has the specific reasons as set by ATO for STP, then have a second box where we can customise our fields eg box 1 - Voluntary Cessation, Box 2 custom - didn't start. Then have box boxes appear on a termination report to give us a greater understanding of why they left

I'd love the ability to be able to have custom reasons, i.e. yes to Tanda's standard reasons, but then we could also create our own.

We have that need to. In our environment, we have people that come in and trial for a full day. We pay them for their contributions on that day, and the only way to do so, is to set them up in Tanda. However, they never actually started, we just needed to pay them their trial shift.
Could an idea be to have an "Organisation Deactivation Notes" section next to the deactivation. To even have a Yes or No for re-hire would be very helpful too in environments with very transient workforces

I think one of the main reasons Tanda's added the 'reason for termination' is for those using Tanda Payroll. I'd assume that when the pay is run, this information is reported to the ATO via STP 2. If so, then the reasons for termination need to match the ATOs standard termination types. They currently do match, so that's why I'm assuming that.
We often have the same thing and what I do is just match their employment end date with the start date that was originally put in (per their contract) and then I select voluntary cessation.
